Simple Form

• First create a table on the data+ server with the fields first_name, last_name, birth_date, used_dataplus_forms.

• Create the needed fields and order them by using new lines:

  

• Set the options for the ButtonGroup:

  

• Select the Db Column (repeat for all fields)

  

• Configure a default value for the birth date & Used Data+ Forms

  

  

• Configure a task reload to use added entries in the app
